Pertinent records can be arranged to be sent to the provider of your choice, to include: Initial Evaluation; last several progress notes; complete medication log; and other items felt to be relevant such as labs, screening forms, etc with a signed and witnessed "Request for Records Release" form (see lower right for forms).  You may then email the completed and executed form to:

conelltelepsych@gmail.com. A response can be expected within 30 days. 

While there is no charge for providing these records to another provider, there will be a charge, as determined by applicable Virginia law, for any special requests for personal copies or additional records.
